Important Information

Each registration provides you with the
following: 1) Access to the teleseminar on a
single telephone line. There is no limit to the
number of people who may listen in and
participate on that line. If you plan to have
people participating from multiple locations on
different phone lines, a separate registration
form and payment must be submitted for each line
used; 2) Each registered site will receive one
(1) set of handout materials supplied by the
speakers. Handouts will be available for
download (PowerPoint or PDF format) at a
specified website approximately 1 week prior to
the teleseminar. Paid registrants will receive
the website address and passcode with their
confirmation information. Teleseminar telephone
number, passcode, and handouts website
information are non-transferable. Only the
actual registrant may dial into the teleseminar
and access the handouts website

If you are unable to download handout materials
and require that paper copies be sent, you must
make your request AT LEAST 5 business
days before the seminar. Handouts will be sent
via UPS Ground for a $7.25 shipping fee.
Requests for handouts received LESS THAN 5
business days from the teleseminar date will be
shipped via FedEx overnight for a $20
shipping fee. Handouts are NOT
available by fax. NO REGISTRATIONS will
be accepted after 3 p.m. Eastern Time, 12/4/08.
50% REFUND until 11/20/08. NO REFUNDS
after 11/20/08. Cancellations must be made in
writing.

Participants phoning in from outside the U.S.
will be assessed a toll (not assessed by NACHA)
in addition to their registration fee based on
their local long distance carrier rates. This
toll will appear on the participant's local long
distance carrier's billing statement.
